Registration and Distribution

I certify that I am the parent or legal guardian of the child(ren) listed on this form, or an adult participant registering on my own behalf, and that the information provided is true and accurate to the best of my knowledge.

I understand that registration does not guarantee any particular item, size, brand, quantity, school supply, backpack, service, or other resource. Items and services are subject to availability and may be distributed on a first-come, first-served basis or according to BCIOI’s reasonable eligibility and distribution procedures.

One registration is permitted per eligible child. Duplicate, false, or materially inaccurate registrations may be cancelled or denied.

Parent/Guardian Responsibility and Participation

Children must remain under the supervision of their parent, legal guardian, or authorized responsible adult unless BCIOI specifically provides otherwise in writing.

BCIOI does not assume custody or guardianship of a child merely because the child is registered for or attends the event.

I understand that participation in a community event may involve ordinary risks associated with crowds, walking, lines, recreational activities, weather conditions, distribution of goods, and interaction with other participants.

To the fullest extent permitted by applicable law, I acknowledge and voluntarily accept the ordinary and inherent risks associated with participation.

Nothing in this acknowledgment is intended to waive any right or liability that cannot legally be waived under New York law.

Medical Emergencies

In the event of an apparent medical emergency involving my minor child and I cannot immediately be reached, I authorize BCIOI representatives to contact 911 or appropriate emergency medical personnel.

I understand that BCIOI does not provide medical treatment and that medical decisions and expenses remain the responsibility of the parent, guardian, participant, or applicable insurance provider.

Donated Items

I understand that backpacks, school supplies, promotional items, or other goods may have been donated by third parties.

Recipients are responsible for reviewing items for suitability, age appropriateness, and other individual considerations before use.

Privacy and Event Conduct

Information collected on this form will be used for event administration, participant communication, planning, distribution, recordkeeping, and other legitimate BCIOI charitable-program purposes.

BCIOI will not sell participant registration information.

BCIOI reserves the right to refuse or discontinue participation when reasonably necessary for safety or orderly event operations, including threatening behavior, harassment, violence, intentional disruption, fraud, or failure to follow reasonable event instructions.

Photography & Media Authorization

YES, I GIVE PERMISSION

I authorize BCIOI to photograph, video record, or otherwise capture the image and/or voice of me and/or my minor child(ren) during the event and to use approved photographs or recordings for legitimate nonprofit purposes, including BCIOI’s website, social media, newsletters, community outreach, fundraising materials, reports, and future event promotion, without compensation.

NO, I DO NOT GIVE PERMISSION

I do not authorize BCIOI to intentionally photograph or record my child for identifiable promotional or marketing purposes. BCIOI will make reasonable efforts to honor this selection.
